Mechanism of action
Pipemidic is believed
to be DNA gyrase inhibitor.pipemidic exhibits bactericidal activity which in
solid media,is not affected by the presence of serum,plasma,red blood cells,heparin
or a change in PH from 5 to 9.
Pharmacokinetics
Protein binding is
27-35 percent.Pipemidic produces urinary levels high enough to eradicate most
sensitive organisms.
Indications
Acute and chronic
urinary tract infections caused by E.coli,proteus,klebsiela,pseudomonas
aeruginosa,enterobacter,citrobacter or staphylococcus.Pipemidic is also
indicated for the treatment of bacillary dysentery and enteritis.
Precautions
Pipemidic is not
recommended for use in patients with severe renal failure (creatinine clearance
below 10 ml/minute).
Side effects
Nausea,abdominal
pain,vomiting,diarrhea and stomatitis.skin eruptions,urticaria,pruritis,fever,fatigue,thirst,leukopenia,headache
and dizziness.
Pregnancy and lactation
In pregnant women and
nursing mothers with respect to adverse effect upon the fetus or newborn has
not been established.
Dosage
For the treatment of
acute urinary tract infections.the recommended daily dosage is 800mg administered
in 2 divided doses for a period of 7-10 days. For the treatment of chronic urinary
tract infections the recommended daily dosage is 800-1600mg administered in 2-4
divided doses for a period of atleast 2 weeks or longer if required.For the
treatment of bacillary dysentery or enteritis the recommended daily dosage is 400mg
administered 4 times daily for a period of 3-6 days.
